Arrive 10 minutes early to complete intake forms.

Bring a valid photo ID and your insurance card if applicable.

Bring a list of current medications and supplements.

For Pap smears, avoid intercourse, douching and vaginal products for 48 hours prior.

Wear comfortable clothing — gowns are provided.
